At Help at Home, our RN Case Managers focus on performing in-home admissions, in-home supervisory visits, office duties, and semi-annual or annual on-site assessments of field caregivers. Our teams are dedicated to ensuring compliance with the client assessment of need and service plan standards that are compliant with state regulations. Work hours for this role are normally Monday – Friday 8:00AM – 5:00PM, but may vary to support business needs.

A typical day for a RN Case manager includes:
Conducting initial evaluation visits and regularly re-evaluating clients’ nursing needs.
Counseling clients and their families regarding nursing needs and related needs.
Coordinating and creating service plans for clients following client assessments and admissions.
Assisting and supervising field staff, ensuring successful implementation of new Medicaid Waiver programs.
Ensuring quality care is delivered to our clients by creating, implementing, monitoring, and enforcing quality assurance policies and procedures.
Assisting our branch teams with pre-service training and orientation as well as in-service training for caregivers.
Assisting with other duties as needed and as assigned by the Branch Manager.
In order to be eligible for this role, you should have:
A current RN license in the state of services
A valid driver’s license
Access to reliable and insured transportation
Ability to travel within the surrounding counties 2-3 days per week between the hours of 8:00am – 5:00pm
Availability to work day shifts
Availability to be On-Call
Nursing experience preferred
